Enrollment is Now Open at
St. John the Evangelist Catholic School
The following requirements must be met in order
for your child/children to be registered.

Registration Fee
- K3 Preschool: $150.00 per child, non-refundable
- K4 Extended Program: $150.00 per child, non-refundable
- K5-8th Grades: $150.00 per child, non-refundable
Registration Form
- Must be completed both front and back, returned with the registration payment, and provide all information requested on the form.
Copy of Birth Certificate
- Children must be 3, 4, 5, or 6 years of age by September 1st to enroll into K3, Voluntary Prekindergarten (K4), Kindergarten, or 1st grade.
Immunization Record
- Blue shot record card completed by a certified Florida Physician or the Escambia County Health Department.
- All shots must be up-to-date before entering school in the fall.
Physical
- A yellow school physical form completed by a certified Florida Physician or the Escambia County Health Department.
